Veterinary referral

If you’d like to make a referral, please fill out the following form. If you do not hear from us by the next working day, please contact us on 015394 88555. If the referral is an emergency please call and discuss with one of our vets before filling in the form.

Please include a full clinical history. For orthopaedic cases, please attach any radiographs of the case.

IMPORTANT: If you are referring a case with suspected cruciate ligament disease please include two orthogonal views of the affected limb. It is important that the lateral view includes both the stifle and the hock (see example below). This is necessary for our pre-operative planning so we can advise you and the client with regard to which surgical option(s) is appropriate.
