Please note this is an information request form for owners interested in our surgery, rehabilitation and dental referral services. To register your pet with Oakhill Veterinary Group as a new client, temporary resident or visitors please click here.

Should you wish to have your pet referred to Oakhill please ask your vets to fill in the vet referral form with the history of your pet attached and any results of investigations such as test results or x-rays. We will then contact you to arrange an appointment or surgery as appropriate.
